OT SESSION COSTS:

Telehealth Session: $100 per session (60 minutes)

In-Home Session: $120 per session (60 minutes)

Zack primarily sees clients via telehealth, but in-home visits can be arranged on a case-by-case basis if preferred. Zack only serves clients in Washington state. For in-home visits, Zack serves the Greater Seattle area and surrounding suburbs. If you live far from Seattle, Zack may only be able to work with you via telehealth. Contact us if you are unsure whether or not you fall within this range. All OT sessions are private pay. A detailed receipt can be provided for you at your request.

CANCELLATION POLICY:

OT Sessions canceled with less than 24 hours notice, or missed without notice, will be charged a cancellation fee ($80 for telehealth or in-home session). Exceptions may be made for emergencies at the therapist’s discretion.

Cancellation timeframe: Cancellations made before 24 hours to the appointment time will not be charged a cancellation fee.

LIFE SKILLS SESSION COSTS:

DDA waiver funding will be accepted for this service. Please refer to your case manager for more information.
